SUMMARYTo provide Diabetes Self-Management Education (DSME) and Medical Nutrition Therapy (MNT) for patients with diabetes and other medical conditions in an outpatient setting
STATUS: Per Diem ~15-20 hours/week
LOCATION: Fully Remote
DEPARTMENT: Food and Nutrition
SCHEDULE: Days
ATTRIBUTES- Bachelor's Degree, with major in Dietetics/Nutrition. MS in Nutrition, Education or Health Preferred OR Associates or Bachelors in Nursing
- Registered Dietitian Nutritionist (RD) through Commission on Dietetic Registration AND Certified Dietitian/Nutritionist (CDN) through NYS Department of Education Or Certified Diabetes Care and Education Specialist (CDCES) within 2 years of hire OR Weight Management Certification within in 2 years of hire OR Licensed Registered Nurse (RN)
- Knowledge of insulin pumps and continuous glucose monitors helpful
- Two to three years of diabetes specific experience with Certified Diabetes Care and Education Specialist credentials preferred or Two to three years weight loss counseling with Obesity and Weight Management certification preferred
- Experience with Bariatric surgery patients helpful
RESPONSIBILITIES- Assesses patients to determine nutritional status and baseline knowledge regarding DSME and MNT in relation to their diabetes or other medical condition.
- Develops and individualized MNT or DSMT complex plan of care appropriate to adolescent, adult, and geriatric patient populations. Uses a variety of complex interventions and methods of behavioral management.
- Collaborates with other members of the health care team to provide Diabetes Self-Management Training (DSME) and Medical Nutrition Therapy (MNT) to maintain the Diabetes Education Program and the Weight Management Program as a center of excellence. May make recommendations to physicians regarding therapies and treatment options.
- Researches trends in Diabetes Self-Management training and Obesity/Weight Management counseling, within scope of practice. Develops and writes curriculum that emphasizes patient self-empowerment and self-efficacy. Assumes the role of educator/mentor by providing goal driven, self-management education and support. Skilled in delivering MNT and DSMT, in both group and 1:1 setting. Monitors educational, clinical and behavioral outcomes and revises practice as needed.
- Conducts managing Diabetes classes utilizing various teaching methods and presenting up-to-date information. Innovates various other class curriculum, topics and settings.
EDUCATION: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. PAY RANGE: $56,160.00 - $83,491.00
